Discover, Visa, MasterCard, and American Express accepted. Convenience fee applies. • Mail: Use Form IT 40P and send to: Ohio Department of Taxation, P.O. Box 182131, Columbus, OH 43218-2131. If you cannot pay, you should still file your extension and pay whatever amount you can manage by April 15. The Department of Taxation will process your file, apply your partial payments, and issue a Notice of Assessment for the remaining unpaid balance. If the assessment remains unpaid after 60 days, the Department of Taxation legally certifies the debt and transfers it to the Ohio Attorney General’s Office (Collections Enforcement Section). The Attorney General is the official collection agency for the state. Establishing the Plan: Once you receive a collection letter from the Attorney General’s Office, you can formally contact them at 888-301-8885 to set up a payment plan.

Do I need to file?

Yes, you must file a federal extension form to be granted an Ohio tax extension To get an Ohio state extension, you must first obtain a federal extension from the IRS using Form 4868. If your federal extension is approved, your Ohio filing extension is automatically granted. When You File: You do not send any extension paperwork to the state in April. Instead, when you file your final Ohio IT 1040 on or before October 15, you must check the box on the return indicating that you filed a federal extension and attach a copy of your federal Form 4868 to the return.
